Cardiac Wellness
Patients who have experienced cardiac events can be referred to the Cardiac Rehabilitation Program, a free, 12-week program that helps to reduce the risk of major adverse cardiovascular events, such as heart attack and stroke. Please call us for program participation or if you have any questions about the content of the videos.
You will work with our team of physician, nurses, dietitian, and pharmacist to learn strategies to manage your cardiac and vascular health. These health outcomes can lessen or eliminate your personal risk factors.

The Cardiac Rehabilitation Program consists of three parts:
1. Assessment
During your appointment, a physician will review your medical history, conduct relevant testing and give you an exercise prescription. After 12 weeks, testing will be repeated, your progress assessed, and further recommendations provided.
2. Education
Education is available in two possible formats.
In the in-person group format option, you will focus on nutrition and medication management. These classes allow participants to ask questions of nurses, a dietician, and a pharmacologist.
In the online format option, you can enjoy the convenience of completing education from the comfort of their home. Please view and complete the education videos below.

3. Exercise
Participation in the exercise program should be done in consultation with your health care provider and at your own risk. Please contact Dr. Martin-Smith for more information.
